• Maintain detailed case documentation and ensure compliance with healthcare regulations.
• Evaluate outcomes and adjust care plans to improve health and cost-effectiveness.

Requirements:

• Active RN license in Illinois.
• Minimum of 2-3 years of clinical nursing experience, preferably in case management, discharge planning, or a related field.
• Strong knowledge of healthcare systems, community resources, and care coordination.
• Exceptional organizational, problem-solving, and communication skills.
• Proficiency in electronic health records (EHR) and remote communication tools.
• Case management certification (CCM) is a plus but not required.
